Patients are scheduled by appointment. On your first visit, a permanent record will be initiated. You will be asked to complete a registration form prior to coming in for your appointment. Be sure to arrive a few minutes early to allow time for the completion of required documentation.

Canceling An Appointment

If you are unable to keep your appointment or if you are going to be late, please call the office as soon as possible. This courtesy allows the office to be of service to other patients. The answering service will relay messages to the staff in case the office is closed.
